I understand that participation in activities offered by OKC Charters involves inherent risks, including but not limited to boarding and exiting the vessel, slips and falls, changing weather conditions, water-related hazards, vessel movement, equipment failure, and actions of other passengers.

I voluntarily assume all risks associated with my participation and release OKC Family Fishing LLC d/b/a OKC Charters, its owners, employees, contractors, and representatives from liability for injury, loss, damage, or claims arising from my participation, except as prohibited by law.

I certify that I am physically able to participate and agree to follow all instructions provided by the captain and crew. I understand that failure to follow safety instructions may result in removal from the charter without refund.

I understand that:

• Children under 12 must wear a life jacket at all times.

• Swimming is not permitted on Lake Hefner.

• The captain may discontinue alcohol consumption if safety becomes a concern.

By signing below, I acknowledge that I have read and agree to the terms above.

Each unrelated adult must complete their own waiver prior to boarding.
